Intrauterine insemination

For unexplained infertility (ovulating regularly naturally or with oral medications, no tubal blockage, no uterine abnormalities, and a normal semen analysis), the first-line treatment is intrauterine insemination with ovulation induction medications and a simple semen wash. 

Day 1: First day of menstrual cycle

Days 3 - 8: Take ovulation induction medication, either clomiphene or letrozole

Day 10: Test for LH surge using at-home urine tests

Next day: Semen is washed with a special solution and centrifuged.  After a check under the microscope, the washed sperm is injected into the uterus using ultrasound-guidance.

 

Chance of success per cycle, by age:

<35 years:    20% 

35-37 years: 15%

38-40 years: 10%

>40 years:    5%​​
